# 8 Observations in Cipla FDA-483

FDA has released the form FDA-483 issued following a 2/6-2/17 inspection at the Cipla sterile and non-sterile drug manufacturing facility in Madhya, Pradesh, India. The eight inspection [observations](https://www.fda.gov/media/165827/download?ref=fdaweb.com) were:

- failing to thoroughly review any unexplained discrepancy and the failure of a batch or any of its components to meet any of its specifications, whether or not the batch has been already distributed;
- failing to establish and follow written procedures designed to prevent microbiological contamination of drug products purporting to be sterile;
- aseptic processing areas are deficient regarding the system for monitoring environmental conditions;
- failing to exercise appropriate controls over computers or related systems to assure that changes in master production and control records or other records are instituted only by authorized personnel;
- procedures describing the handling of written and oral complaints related to drug products are deficiently written or followed;
- a timely Field Alert Report was not submitted following receipt of information concerning a failure of one or more distributed batches of a drug to meet the specifications established for it in the application;
- failing to fully follow the responsibilities and procedures applicable to the quality control unit; and
- failing to include in laboratory records complete data derived from all tests, examinations, and assays necessary to assure compliance with established specifications and standards.
