FDA has released a [draft guidance](https://www.fda.gov/media/188891/download?ref=fdaweb.com) outlining methods drugmakers should use to collect long-term safety and efficacy data once cell and gene therapy products are approved. The recommendations reflect growing concern over how to monitor treatments that may have lifelong effects but are tested in relatively small patient populations before reaching the market.

The document, *Postapproval Methods to Capture Safety and Efficacy Data for Cell and Gene Therapy Products*, emphasizes that long-term follow-up is critical to understanding the durability of treatment benefits as well as risks such as delayed side effects, cancer, or mortality related to the therapy or underlying disease. FDA notes that pediatric patients present unique challenges, often requiring data collection into adulthood and new consent procedures as patients age.

Among the methods highlighted, FDA encourages sponsors to use real-world data sources such as electronic health records, claims data, and digital health technologies, provided they include strong data governance, privacy protections, and quality controls. Additionally, it recommends registry networks to collect standardized, longitudinal data, including patient-reported outcomes and genetic or imaging data, to supplement clinical trials.

FDA says robust postapproval data can help balance limited premarket evidence, particularly for therapies approved under accelerated pathways. Such data may inform regulatory decisions, clinical practice, and future drug development, it adds.
