A 4/1-4/5 FDA Bioresearch Monitoring Program inspection at a clinical trial site operated by neurologist **Han Phan** in Atlanta, GA, found that she did not adhere to applicable laws and regulations governing the conduct of clinical investigations. A 12/3 [Warning Letter](https://www.fda.gov/inspections-compliance-enforcement-and-criminal-investigations/warning-letters/han-c-phan-md-696989-12032024?ref=fdaweb.com) says Phan failed to ensure that the investigation was conducted according to the investigational plan.

Phan was told to respond with a list of actions she has taken to prevent similar violations in the future.
