Cook Fixed Core Wire Guide Recall Class 1

FDA says the Cook Medical 10/14 recall of its fixed core wire guide is Class 1. An agency statement says Cook recalled the product because five lots of wire guides dropped onto an unchecked surface during production. The notice said the drop could cause the wire guide to bend out of shape or become dirty.

“Use of the affected product may cause serious adverse events,” FDA says, “including end-organ embolization.” The notice says there have been no complaints about the device issue and no reported injuries or deaths.

The wire guide is a curved stainless steel wire with a non-reflective coating that helps insert and guide the placement of medical devices like catheters and introducer sheaths inside a patient’s body, the notice says.
