FDA Weighs ‘Black Box’ Warning on Covid Vaccines

FDA is preparing to add a Boxed Warning — the agency’s most stringent safety alert — to Covid-19 vaccines, according to two people familiar with the deliberations. The potential move marks a dramatic escalation in the Trump administration’s ongoing campaign to scrutinize and restrict use of the vaccines, which were among the most widely cited public health successes of the pandemic era, according to a CNN report.

The plan, which has not been finalized, is reportedly being advanced by CBER director Vinay Prasad. Key details remain preliminary, including whether the warning would apply only to mRNA vaccines, or to all licensed Covid-19 vaccines, and whether it would apply across all age groups. The warning could be announced before the end of the year, CNN says.

FDA’s ongoing actions are troubling public health experts, who say no such evidence exists for Covid-19 vaccine restrictions and increased warnings. “Unless the FDA announces it, any claim about what it will do is pure speculation,” said HHS spokesperson Andrew Nixon. But the reports have already triggered significant alarm among former FDA leaders, academic researchers, and vaccine-safety specialists who warn the agency appears to be bypassing normal scientific processes.

Moderna and Pfizer, whose mRNA vaccines make up two of the three FDA-approved Covid-19 shots, reiterated their earlier statements affirming robust, global safety monitoring, CNN says. Moderna noted that more than one billion doses of its SpikeVax vaccine have been administered worldwide without new or undisclosed safety signals in children or pregnant women. Pfizer likewise restated its position on the vaccines’ strong safety and efficacy profiles and declined to comment further.

The safety record has been widely validated. A CDC analysis released 12/11 found that updated Covid-19 vaccines used during the 2024–25 respiratory virus season reduced emergency-department and urgent-care visits by 76% in children ages nine months to four years, and by 56% in children ages five to 17. Earlier research estimated that Covid-19 vaccination prevented nearly 20 million deaths globally in its first year.

The push for a boxed warning comes amid internal volatility at the agency. Last month, Prasad circulated an internal memo claiming the agency had identified “at least 10” pediatric deaths “after and because of” Covid-19 vaccination, without providing data. He called for “swift action.” According to HHS, the agency is reviewing potential safety issues “across multiple age groups,” but has released no supporting information.

Several experts said they were unaware of any advisory committee meetings, public notices, or manufacturer-initiated label changes, which are the usual steps when FDA considers a Boxed Warning. “There isn’t that same opportunity for discussion and good-faith review of the data,” Aaron Kesselheim of Harvard’s Program on Regulation, Therapeutics and Law. “My concern is that this will be seen as yet another in the long line of decisions that this FDA is making based on its own political goals.”

Whether the FDA ultimately proceeds with a boxed warning remains unclear, according to CNN. For now, the prospect of a top-level FDA warning without the usual scientific process has heightened tensions around one of the most scrutinized medical products in history and raised fears of long-term harm to public confidence in vaccinations.
