# Hospira Recalls Vancomycin Lot for Particulate Matter

Pfizer’s Hospira unit is recalling one lot pf vancomycin hydrochloride for injection, 750 mg/vial, following a confirmed customer report of the presence of particulate matter, confirmed as glass, in one vial. A Hospira [notice](https://www.fda.gov/Safety/Recalls/ucm574258.htm?ref=fdaweb.com) published by FDA says the company has not receive reports of any adverse events associated with the issue for the lot.

The lot in question contains 10 units and was distributed from 8/2016 through 1/2017 nationwide in the U.S. and Puerto Rico. The company says it has initiated an investigation to determine the root cause of the problem and corrective and preventive actions.
