Info Collection Revision on Expanded Access Form

Federal Register notice: FDA has submitted to OMB an information collection revision entitled “Expanded Access to Investigational Drugs for Treatment Use” (OMB Control Number 0910-0814). The agency has proposed revisions to Form FDA-3926, Individual Patient Expanded Access Investigational New Drug Application. For example, it proposes to move from Field 8 to Field 1 the data fields for “Physician Name, Address, and Contact Information.” Additionally, FDA is adding “Race and Ethnicity” as an optional item under the “Clinical Information/Brief Clinical History” field.
