**Lee Silsby Compounding Pharmacy** Cleveland Heights, OH (7/10): FDA inspected the firm 6/2017 and found that it was not adhering to drug compounding regulations. Investigators also noted serious deficiencies in its practices for producing sterile drug products, which put patients at risk. For example, investigators observed an operator sitting with her upper body leaning into the ISO-5 classified area with the sleeves of her non-sterile gown resting directly on the work surface and thereby providing a potential source of contamination.
