# Olympus Recalls Endoscope Guide Sheaths

Olympus is recalling certain models of its endoscope single use guide sheath kits after receiving complaints about the radiopaque tip falling off into patients. “Olympus’s preliminary findings show that the tip detaches from the guide sheath when excessive force is applied as instruments are inserted into the guide sheath, and/or if there is damage to the distal end of the sheath,” a [recall notice](https://www.fda.gov/medical-devices/medical-device-recalls/endoscope-instrument-recall-olympus-removes-single-use-guide-sheath-kits-due-risk-radiopaque-guide?utm%5Fmedium=email&utm%5Fsource=govdelivery) says. “The use of affected product may cause serious adverse health consequences, including bleeding and death.” To date, the notice says there have been 26 serious injury reports.

The kits are designed for use with an Olympus endoscope to collect cells or tissue specimens in respiratory organs.
